Public health in the 21st century is facing complex challenges that require social change with fundamentally new approaches to succeed. Public health practitioners can address these challenges by viewing their work in a larger social context, comprehending local and global issues in a deeper level, and collaborating with the community in a meaningful engagement. The successful implementation of health policies aimed at reducing health inequities and health inequalities lie in the ability of public health practitioners to integrate multiple disciplines and expertise, employ a systems thinking approach, and more importantly, collaborate with other health practitioners, policymakers, researchers, community members, and concerned stakeholders.

Chung-Sy Inseptum, Inc. is committed to provide leadership and excellence in utilizing interdisciplinary and multidisciplinary health initiatives to high risk individuals, families, and communities; educating and training of medical, nursing, and other healthcare professions; and improving the overall health status of the community through community-based public health approaches.

Kidney Life Specialty Clinic and Dialysis Center will be a locally-recognized interdisciplinary and multidisciplinary kidney center by 2022.

MISSION

We are committed to facilitate positive social change through the design and implementation of Chronic Kidney Disease health initiatives based on the strengths and existing resources within the community and tailored according to the needs of the targeted individuals and families in order to increase their capacity to self-manage their health conditions and for better health outcomes.

GOAL

Improve the overall health status of the community through quality hemodialysis treatment and community-based Chronic Kidney Disease health initiative.

OBJECTIVES

  • Identify high risk individuals through health screenings and case findings.
  • Prevent and/or delay Chronic Kidney Disease progression through smoking cessation, weight maintenance, and diet modification.
  • Promote health and well-being through proper nutrition and increased physical activity.
